Position Overview:
SCOPE OF ROLE:

As part of the ACT Teams the LPN will take vital signs, such as blood pressure, temperature and weight, follow up with community health care providers to ensure medical treatment plans are documented, monitored and followed, and provide CPR/First Aid.

 

ESSENTIAL DUTIES & RESPONSIBILITIES:

The LPN assesses, monitors, and ensures access to coordinated medical and mental health services.

This position requires you to conduct home visits on a regular basis with 50% fieldwork/travel. You will work with consumers in filling prescriptions, coordinating appointments refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment, and discharge planning with hospital staff.

The position requires maintaining up to date, accurate, complete, and timely consumer chart documentation, attendance at staff meetings, case conferences, group supervision, and trainings as needed.

You must comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.

Additionally the LPN will assist the RN, perform record keeping as per agency regulations, and other related duties as assigned. 

The salary min $60,000 to max $60,000.

Qualifications:
 

REQUIRED EDUCATION AND EXPERIENCE:

New York State Licensed Practical Nurse

Good written and oral communication skills required

An ability to work independently as well as part of an interdisciplinary team

 

PREFERRED QUALIFICATIONS & SKILLS:

Experience in direct clinical work with psychiatrically disabled populations preferred

Bilingual English/Spanish speaking preferred
