Job Requirements / Description
Position Overview:
The LPN will;
Conduct home visits on a monthly basis visiting up to 25 consumers
Work with consumers in filling prescriptions and coordinating appointments as needed.
Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
Maintain CPR/First Aid Certification.
Maintain up to date, accurate, complete, and timely consumer chart documentation.
Attend staff meetings, case conferences, group supervision, and trainings as needed.
Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
Assist full time Registered Nurse
Record keeping as per agency regulations
Related duties as assigned
The minimum is $33.65/HR and the maximum is $34.33/HR

Qualifications:
New York State Licensed Practical Nurse
Experience in direct clinical work with psychiatrically disabled populations preferred.
