S:US is seeking a Health Counselor for our women's shelter in Manhattan
This position is located at : 205 W 135th St New York, NY 10030
The Health Counselor is an Administrative, Case Management role.
Salary:
Minimum Salary:$19.23/hr.
Maximum Salary: $20.01/hr.
In this position you will:
Identify individuals who know they are HIV-positive but are not in HIV medical care by receiving referrals, utilizing organizational information about missed appointments, and conducting outreach at various community organizations and venues
Identify those at high risk and have not been tested
Enrolling individuals into program
Completing intake forms (GPRA, local assessments)
Obtaining medical baseline information: lab results, name of primary care provider
Administering health promotion curriculum and track progress
Meeting with clients after each HIV/HCV primary care appointment
Developing and implementing an Individual Care Plan
Providing intensive case management utilizing Motivational Interviewing and Anti-Retroviral Treatment and Access to Services (ARTAS)
Co-facilitating support groups and ensure client linkage to psychosocial support
Help plan, communicate and coordinate behavioral health and medical care needs
Monitoring HIV treatment and communicate with medical providers to document viral suppression status
Conducting HIV/HCV oral testing
Conducting pre and post test counseling
Maintain a caseload of 10-20 clients
Comply with all program protocols, including those regarding confidentiality of client information
Complete employee orientation and all mandatory program trainings to guarantee appropriate competency in program requirements
Qualifications:
Bachelor’s Degree in human services or related field required
A minimum of two years’ experience working with people who have a mental illness
Computer literate in word processing and excellent writing capabilities to develop treatment plans.
